The Social Worker/Case Worker provides home-based, intensive, strength-based, comprehensive services using the Solution-Based Casework (SBC) framework.   Our superstar Social Worker candidate is: - Passionate about providing high quality services to families in child welfare - A lifetime learner, continuously developing their clinical skills - Dedicated to modeling strength-based, trauma-informed practice with families and colleagues. - Culturally humble with a commitment to the Good Shepherd Services mission, vision, and values   Major Duties of Social Worker: Direct Work with Families - Provide home-based and on-site individual, family, and group counseling, as well as short-term crisis intervention, including advocacy and outreach for designated caseload of eight (8) families. - Engage families immediately upon referral, at times participating in Administration for Children’s Services (ACS) Child Safety Conferences. - Implement and integrate the principles and procedures of Solution-Based Casework (SBC): assessment, family and individual level outcomes, action plans, and documentation and celebration. - Handle case-related crises with professionalism, urgency, and care, including incidents involving reports to the State Central Registry. - Conduct screenings and assessments for substance use, mental illness, and child safety and well-being, including assessing caregiver capacity, home environment and conditions, discipline methods, etc. - Integrate the practices of SBC into every counseling session. - Ensure that every child has been seen and properly assessed every month. - Meet appropriate case work contacts for each phase of treatment: Initial, Baseline, and Stabilization. - Represent participant needs, concerns, and perspectives at all required ACS and GSS meetings and trainings.   Administrative and Advocacy Role - Collaborate closely with ACS, alcohol and other drug treatment providers, schools, medical and mental health clinics, and other resources serving family members. - Provide consultation to and collaborate with Family Workers as needed to ensure high quality services to families. - Collaborate with ACS Family Services Unit (FSU) staff, Family Court Legal Services (FCLS) staff and other service providers in making safety and treatment decisions. - Attend and participate in Family Court hearings for families involved with ACS FSU. - Facilitate referrals to alcohol and substance abuse treatment facilities, mental health treatment providers, and other service providers, including escorts to appointments as needed. - Actively participate in Clinical Diagnostic Team meetings (CDTs) to provide case information, updates, and assessments in order to obtain recommendations for continued case practice or case termination. - Participate in case consultation, continued SBC training, and the SBC certification process. - Prepare and maintain FASP documentation, monthly statistical reports, and case work notes in adherence with agency policy. - Provide Court reports as needed to inform as to families’ progress or lack of progress in achieving outcomes. - Maintain appropriate utilization of state and city databases including PROMIS and Connections, as well as other agency databases as needed.   Qualifications: - Master’s  in Social Work Required OR Master's in Mental Health Counselling - LMSW eligible candidates highly encouraged to apply! We are looking for a dynamic and compassionate Case Planners to work with us. In this role you will provides comprehensive case planning services for therapeutic children/youth and their families.  In addition, this person will engage both youth and families in a problem solving process that leads to an appropriate plan of permanency.  In addition there will be a focuses on safety, permanency planning with adult connection and PYA life skills development. Participate in interdisciplinary team model practice, which will involve the Sociotherapist, Health Services, Voc/Ed Specialist and Preparing Youth for Adulthood Specialists.  The Case Planner monitors the Treatment foster home to ensure safety, well being and the development of an individualized behavior plan. He/She subscribes to the concept of positive discipline and behavior modification as effective components in treatment modalities for behavioral difficulties with emotionally disturbed children and imbeds the Sanctuary 7 commitments and tools into trauma informed practice.   Reports To: SOCIAL WORK SUPERVISOR, TREATMENT FAMILY FOSTER CARE   Location: 7 West Burnside Avenue, Bronx, New York 10453   Hours: 35 Hours/Week; Non-Exempt   Major Duties: - Conduct a face to face contact with newly admitted youth and their treatment parents, within 72 hours and weekly mandated home visits thereafter to provide comprehensive case planning services. - Conduct mandated face to face home visits/contacts with birth parents and/or discharge resources for the provision of permanency, case planning and discharge planning. - Complete required paperwork, including but not limited to FASPS, Plan Amendments, Permanency Hearing Reports, life skills assessments and progress notes. - Keep case records up-to-date with the status of case activity, including progress notes on all contacts. - Make all court appearances related to cases, representing the agency as to the permanency, safety and well being of participants served by the program. - Set-up, monitor and document all family and/or sibling visits. - Provide direct crisis intervention as needed.   - Collaborate with Treatment Parents, Youth, and Socio-therapists around development and implementation of behavior plans and goals. - Attend and participate in all  applicable ACS meetings, transfer conferences,  Treatment Team Meetings, Monthly  Shared Quarterly Meetings and monthly Staff Meetings  - Coordinate and participate in all applicable Family Team Conferences.  - Attend Preparing Youth for Adulthood Workshops with youth on a rotating basis. - Gather social history, to assess family strengths and weaknesses, and to assess existing problems to develop an individualized and culturally appropriate service and behavioral plan. - Work with family and youth to develop the most appropriate permanency plan with emphasis on lasting family connections, which can be, improved family relationships, adoption, mentor , etc - Work with the Adolescent Services team for the comprehensive service planning for each youth, including education, employment and other PYA skills. - Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care, when applicable, in the department/program to create a non-violent and trauma-sensitive culture. - Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture. - Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment - Locate resources and make referrals as needed.  These include consistent outreach and referrals in the community.
