Overview

The Nurse Supervisor is responsible for the oversight of the medical and health needs of youth in Therapeutic Family Foster Care and to supervise TFFC Nurse Care Managers and Health Services Assistants. In addition to serving as the Nurse Care Manager for Green House Residential Program. Promotes and restores the health of youth by completing the nursing process, collaborating with a multidisciplinary team. Provide physical and psychological support to youth and their families.

 

Reports to:                          Director of Nursing, Family Foster Care

Location:                             7 West Burnside Avenue, Bronx, New York 10453

Hours:                                 35 Hours/Week; Exempt

 

Major Duties:

  • Provides supervisory support to assigned Care Management Team (includes TFFC Nurse Care Managers and Health Services Assistants)
  • Supervise team in their management of assigned cases
  • Provides individual and group supervision to assigned Care Managers and evaluates all supervisees on a regular and timely basis
  • Monitors assigned Care Managers’ work activities with children and families through team meetings and regular individual meetings
  • Provides direct care management services in the absence of Care Management staff
  • Manages, reviews, and monitors maintenance of member records to ensure both internal and external quality assurance standards and timely completion in accordance with agency guidelines
  • Maintain and monitor 29I documentation and 29I program requirements
  • Provides training to new staff and on-going training to existing staff.
  • Monitor required staff trainings and make sure they are completed in a timely manner.
  • Demonstrates strong leadership and management skills to ensure progressive, positive development of supervisees
  • Coordinates and conducts plan for audit preparation and audit execution
  • Collects, analyzes, and reports data in the area of supervision to assist the Director of Nursing
  • Oversees the identification of eligible members from other agency programs and the community for program enrollment and consenting processes
  • Assists in locating and creating support networks in the community
  • Works collaboratively with all teams throughout agency programs
  • Promote client’s well-being by establishing goals and providing education to understand medical conditions, medications, and therapeutic interventions.
  • Coordinate and implement on going health education programs for youth in our care
  • Lead the implementation of the Sanctuary Model of trauma-informed practice within the department/program.
  • Execute duties of Nurse Care Manager for Green House Residential Program
  • Completes special projects and assignments as needed or identified by the Director
  • Participate in on-call Nurse rotation in case of emergencies (differential pay)
  • Other duties as assigned

 

Qualifications:

  • Minimum LPN Degree in Nursing, plus appropriate licensure
  • Current American Heart or American Red Cross CPR Certification for infants, children, and adults
  • Must demonstrate firm knowledge of nursing principles and practices
  • Knowledge of best practice working with children and families in health, preventive and wellness care
  • Working knowledge of child welfare, developmental disabilities, mental health and/or primary healthcare fields
  • Experience managing teams providing case/care management
  • Experience using Excel and electronic health care systems to review and manage data preferred
  • Experience providing service coordination and information, linkages, and referrals for community-based services
  • Excellent accountability, writing, communication, and organizational skills
  • Compassion for serving children and families with special needs
  • Ability to travel to community-based agency offices
  • Proven decisiveness, motivational, and developmental skills; management abilities, including meeting deadlines, ensuring compliance with agency policies and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirement
  • Valid driver’s license preferred
